PREPARING FOR YOUR VISIT:
Please obtain copies of previous laboratory testing, imaging reports, pathology reports and other pertinent medical records prior to your initial consultation. It is most effective to send a copy to our office via fax in advance of your visit so that the provider may review your records prior to your consultation. We also recommend bringing a hardcopy to the visit with you. Please bring all medication bottles to your initial visit.
Please arrive 15 minutes prior to your scheduled visit to allow time for our check-in process. This will allow you to spend more time with the provider to discuss your condition and treatment plan and will also help our clinic to run smoothly and efficiently.
If you have diabetes, please bring your glucose meter and/or records of your recent blood sugar readings to every appointment.
Please bring your insurance card and co-payment to each appointment. We accept cash, checks and all major credit cards.
